Yul Brenner pulled it off in The King and I. Chris Daughtry of "American Idol" fame and Vin Diesel in the Fast and the Furious series make him look downright sexy. But for most of us, especially women, a paste bald or thinning, sporting is a source of intense distress.
The American Academy of Dermatology estimates that about 30 million women and 50 million men in this country suffer from hereditary hair loss. Many others still lost hair as a result of health or drug problems. But "the vast majority of permanent hair loss is baldness male or female," said Jody Levine, MD, Director of the dermatology plastic surgery and dermatology in New York City.
Causes of hair loss
Man or woman, or androgenetic alopecia, hair loss says Dr. Levine, is a hereditary condition that makes them more sensitive to testosterone hair follicles (women have hormone male testosterone, too, although a lesser amount than men). In these individuals, produced by hair follicles, called 5-alpha-reductase, enzyme converts testosterone into its active form, dihydrotestosterone (DHT). DHT attacks hair follicles, causing hair to fall eventually.
Although the hereditary hair loss mechanism is even hair loss, male and female takes on different models. Men tend to have a receding hairline at the forehead or Crown, with a fringe of hair left in the back and the temples. They can become completely bald. Most women have thinning diffuse, but especially at the top of the head and rarely experience total baldness.
Not all hair loss is hereditary, or permanent. Indeed, a loss of hair is perfectly normal. Every day you lose about 100 100 000 to 150 000 hair on your head as part of the normal life cycle of hair follicle. Minoxidil (Rogaine) is a topical medication that is sold over-the-counter. Used by men and women, it works by increasing the amount of time that your hair follicle is anagen (growth). Most users of Rogaine notice a slowing of hair loss rate in addition some shifts, although the new hair may be thinner and shorter than before. Rogaine is applied to the scalp twice a day and lasts about three months for best results.
Finasteride (Propecia) is a drug of the description that is taken orally. It is not approved for use by women of child-bearing because it can cause birth defects. PROPECIA works by blocking the enzyme that converts testosterone to DHT, slowing the hair loss and promote hair regrowth. It can take several months to see the full effect of Propecia.
Corticosteroids can be injected into the scalp to treat certain types of hair loss such as alopecia areata, considered an autoimmune problem. It takes about a month for the hair to start following treatment with cortisone.
'Off-label' of drugs or drugs approved by the FDA for another condition, but which inadvertently found to help hair loss, can be used to treat difficult cases. Dutasteride (Avodart) is one of these drugs. Currently used to treat benign prostate hypertrophy, it decreases the amount of testosterone is converted to DHT. Research suggests that the Avodart lowers as much as 90 percent of DHT in the blood, an improvement on the 65 to 70% reduction of DHT with Propecia.
Other medicines on the label used for female pattern baldness include inhibitors of androgen receptors, such as Spironolactone (Aldactone). Aldactone is a diuretic (water pill) that works for hair loss by preventing the cells of the body to interact with androgens (testosterone), the reduction of the follicle, damaging the DHT.
Low level Laser Therapy (LLLT) has been used for years in medical settings for temporary pain relief and healing of wounds and is also offered in some contexts of Dermatology for hair loss. This treatment involves sitting under a hood device that contains panels of lasers that shine on the scalp, deliver some wavelengths of light during term-time to the area. While he knows exactly how LLLT helps hair loss, David Michaels, inventor of a product approved by the FDA of Pocket called HairMax LaserComb, laser indicates that the mechanism of action is "transfer light to cellular energy energy" into the hair follicles.
Research has shown that some people indeed benefit from laser treatment for the loss of hair, either by cell stimulation or increased blood flow to the treated area. It may take longer than six months to see good results with the laser treatment, and as a treatment drug, LLLT must be used continuously to maintain the growth of hair. "It is more effective in the early stages of hair loss," says Michaels. "It is not for someone who is totally bald."
Hair Restoration Surgery is permanent only for hair loss, said Nicholas Nikolov, MD, FACS, a plastic surgeon in Beverly Hills. Early surgery of hair involved round sections (grafts) of 12-20 hair restoration each payroll head and transplanted to the bald area, explains Mr. Nikolov. But it was difficult to maintain blood flow to the graft, and often the hair in the middle of the round graft would die, causing a "donut" appearance. Many hair restoration surgeons make unit follicular transplantation by removing a strip of hair follicles (with skin) on the back of the head, where the hair are genetically programmed to remain the longest. The grafts are then transplanted into small incisions in areas of baldness, and the 'donor' area is sewn together.
Mr Nikolov uses another technique for harvesting of grafts, called follicular units automated extraction (FUE). This method, unlike the method of the band, removes follicles separately from the donor area using a small automated punch and forceps. Then the grafts are microscopically divided and implanted in the same manner as the method of the band. The advantage of FUE, said Mr. Nikolov, is that there a no incision and suture the donor site, which means an easier recovery and less scarring. The downside: it takes about once and a half longer for the procedure.
Any method of follicular unit transplantation, it is a (5-8 hours) long and expensive process. Of post-operative instructions to follow, and need to wait about six months to see the full results (period during which newly transplanted hair fall before the hair regrowth). In addition, few women is good candidates for follicular unit transplantation, because unlike men, they rarely hair follicles healthy at the back of the head for grafting of donors.
Expectations of treatment
